Sign in to CVS.com or the CVS Pharmacy app using the account your ExtraCare Plus (CarePass) membership is attached to.

Open your account and go to ExtraCare Plus / CarePass membership management (Manage membership).

Start the cancellation flow; if you are shown a reason prompt or a 'stay and save' retention offer, decline it or pick a reason to continue.

Confirm the cancellation. Do this at least 3 days before your renewal date to avoid the next charge.
