Subscription and Gym Cancellation Letter Template
A cancellation letter creates a dated, written record that you asked to end a subscription or gym membership. Include your account number, a clear cancellation statement, the effective date, a request to stop recurring charges, and a request for written confirmation. Send it by a trackable method, keep a copy, and check your next one or two statements.
When a cancellation letter still matters
Most streaming and app subscriptions now cancel in a few taps, so a formal letter is not always necessary. But a written cancellation still matters in several situations: gym and health-club memberships, any contract with a required notice period (often 30 days), and services billed directly from your bank account or by paper invoice. A dated letter creates a paper trail — the record that protects you if the company keeps charging or later disputes when you canceled.
One caveat: subscriptions purchased through the Apple App Store or Google Play are canceled in your Apple Account or Google Play settings, not by writing to the company. Deleting the app does not stop the billing. For those, cancel in the platform's account settings and keep the confirmation.
What to include in the letter
A cancellation letter can be short, but it should be unambiguous. Every version should contain the same core details so the company cannot claim it did not know what you wanted or when.
Include all of the following: - Your full name, mailing address, phone, and email - Your account, membership, or contract number - The date you are writing - A clear statement that you are canceling - The effective date (or the date any required notice period ends) - A request to stop all recurring charges and not renew - A request for written confirmation of the cancellation - Your signature
For a gym or any signed agreement, add one more thing: a reference to the contract's cancellation clause, and — if you are relying on a specific legal right to cancel — a short note stating which one and enclosing any supporting documentation.

Gym or health-club cancellation letter template
Gym contracts usually have stricter rules than app subscriptions, so name the contract and the notice terms explicitly.
[Your name] [Street address] [City, State ZIP] [Email] | [Phone] Membership / contract #: [number] [Date] [Gym name] [Gym or corporate address — check your contract] Re: Cancellation of membership — contract #[number] To whom it may concern: I am writing to cancel my membership under the contract above and to end all recurring billing. Please treat this as written notice under the cancellation terms of my agreement, effective at the end of any required notice period. [If applicable: I am exercising my right to cancel because I have moved more than [X] miles from the facility / I am medically unable to use the facility / the facility has closed or relocated. Documentation is enclosed.] Please confirm the cancellation and my final billing date in writing, and refund any amounts charged after that date. Sincerely, [Signature] [Printed name]
Many states regulate health-club and gym contracts and grant members specific cancellation rights — for example, a short cooling-off period soon after signing, or the right to cancel if you move a set distance from the facility, become medically unable to use it, or the club closes or relocates. The exact conditions, distances, and required notice method vary by state, so check your state's health-club law and your own contract before you send the letter, and enclose proof (a lease, a doctor's note) when you rely on one of these rights.
How to send it and confirm the charges stop
First, read your contract for the required cancellation method and window. Some agreements demand written notice sent to a specific address, and count the notice period from the day they receive it. Send the letter by a trackable method — certified mail with return receipt is the standard for gym contracts and any likely dispute — and keep a copy of the letter and the mailing receipt. If you also call, write down the date, the representative's name, and any confirmation number.
Then watch your bank or card statements for the next one or two billing cycles to confirm the charges actually stop. A cancellation is not complete until the billing ends.
If a company ignores the letter and keeps charging you, you have further options. You can revoke your authorization for the recurring payments and ask your bank to place a stop-payment order; the CFPB explains this process. You can also dispute the charges with your debit or credit card issuer, which the FTC recommends when a company will not stop billing after you cancel. Note that stopping a payment does not by itself cancel a valid contract or erase money you legitimately owe — so keep resolving the underlying agreement in writing.

FAQ
Do I have to send a cancellation letter by certified mail?
Not always — check your contract first, because some agreements require written notice sent a specific way. Certified mail with a return receipt gives you proof of delivery and the date, which is worth it for gym contracts or any situation where the company might dispute when you canceled.
Can I stop a company from charging my card if my letter is ignored?
Yes. You can revoke your authorization for the recurring payments and ask your bank to place a stop-payment order (the CFPB describes how), and you can dispute the charges with your card issuer. Keep in mind that stopping payment does not cancel a valid contract or erase what you legitimately owe, so continue resolving the agreement in writing.
Does a letter cancel an App Store or Google Play subscription?
No. Subscriptions billed through Apple or Google are canceled in your Apple Account or Google Play settings, and deleting the app does not stop the billing. Cancel there to stop the renewal and keep the on-screen confirmation; a letter to the company will not stop platform billing.
